We make it simple to get started with us. If it’s your first time, you can fill out your new patient forms on one of our tablets while you wait. Just bring a photo ID and a copy of your insurance card to your appointment if you’ll be using insurance. Or, get a jumpstart on your forms by clicking below to fill them out ahead of time.

New patients can take advantage of our $125 new patient special. You receive a thorough dental exam, teeth cleaning, and a set of x-rays. Dr. Burke will examine your teeth and gums and address any concerns you may have. If any further treatments are recommended before your next routine exam, you’ll work together to discuss the best options for your smile.

What are my options for missing teeth?

There are many options for replacement of missing teeth, but these vary from patient to patient. During our examination, we will look to see what your options are that would serve you best!

How can I pay for my appointments?

We offer payment options, including an in-house Smile Club Membership for non-insurance patients. We will be happy to discuss those options further if treatment is necessary.

How often should I see the dentist?

For preventive care, you should come in twice a year for a dental exam. During these appointments you’ll receive a thorough cleaning, an examination to identify any issues with your gums or teeth, and x-rays to catch any cavities. Dr. Burke will talk with you to discuss any concerns you have about the look or health of your smile—and what your options are.
